Macomb nursing home fined by state

Countryview Care Center, 400 W. Grant St., Macomb, was fined $25,000 by the Illinois Department of Public Health.

A Macomb nursing home has been fined $25,000 for failing to properly administer medication to a resident with type 2 diabetes.

The Illinois Department of Public Health fined Countryview Care Center, 400 W. Grant St., for a Type A violation. That’s for incidents with substantial probability of death or serious mental or physical harm.

State investigators visited Countryview Care Center in May. They say the staff’s failure to properly administer medication elevated the patient’s already high glucose level, which led to the resident being found unresponsive in the nursing home dining room.

The resident was taken to a local hospital. In the emergency room, the resident’s blood glucose level was 713 mg/dl. A level of 200 mg/dl is considered high.

The resident was later taken to a tertiary hospital for treatment of septic shock, acute kidney injury, and hyperglycemia.

The violation was detailed in IDPH’s third quarter report.

Countryview Care Center was also fined for Type A violations in 2022 and 2023.

The nursing home is licensed to Petersen Health Properties, which is affiliated with Peoria-based Petersen Health Care.

The company filed for Chapter 11 bankruptcy in March.
