State fines Galesburg nursing home $50K over missed medications, neglect

State regulators have fined Marigold Rehabilitation & Health Care Center $50,000 following an inspection that identified serious violations of resident care standards, including failure to administer prescription medications over multiple days.

The state issued a Type AA violation against the Galesburg nursing home, which is the most severe classification, indicating conditions that create a substantial probability of death or serious harm.

According to the Illinois Department of Public Health, the violations were documented during a Jan. 9, 2026, survey at the facility, 275 E. Carl Sandburg Drive, and were related to the care of a resident who died in October 2025.

Inspectors found the facility failed to ensure the resident was free from neglect by failing to provide prescription medications in accordance with physicians’ orders, despite the medications being available on-site. They also found the facility failed to assess the resident and intervene. As a result, the resident experienced a decline in condition and subsequently died.

The inspection report states the facility’s failure to follow physicians’ orders and respond appropriately put the resident at harm for serious risk and resulted in actual harm.

The facility is licensed to Marigold HCO, LLC, based in Springfield.
