A Galesburg nursing home has been fined $25,000 by the Illinois Department of Public Health following an investigation into a non-consensual sexual relationship between a former dietary aide and a 28-year-old nursing home resident with disabilities.
Surveyors found Allure of Galesburg, 1145 Frank St., failed to protect the resident from sexual abuse and provide adequate supervision. The nursing home also failed to properly investigate and report the abuse allegations, leading to the Class A licensure violation.
The investigation found the dietary aide was hired in September 2023 and resigned in June 2025, after being confronted by nursing home administrators about texting with the resident.
After resigning, the former dietary aide continued to contact the resident and send her sexually explicit text messages. The investigation found he sexually assaulted the resident three times in his vehicle while attending church services in November 2025.
IDPH conducted a complaint investigation in February 2026, leading to the fine.
This is the second year in a row Allure of Galesburg has been fined for failing to protect a resident from sexual abuse by a staff member.
In 2025, surveyors found a certified nursing assistant repeatedly sexually abused a male resident over a seven-month period beginning in June 2024. The nursing assistant coerced the resident with alcohol, THC products, and e-cigarettes, according to the investigation.
The male resident later sued Allure of Galesburg and the nursing assistant in Knox County circuit court. He’s seeking more than $50,000 under the state’s Gender Violence Act.
In addition, surveyors found in June of this year another instance of the facility failing to implement its abuse policy. According to that investigation, employees failed to report months of rumors that a certified nursing assistant was having an improper sexual relationship with a resident during his stay and after he was discharged.
No state sanctions have been filed in that case at this time.
Skilled nursing facilities such as Allure of Galesburg can admit adult patients of any age recovering from severe trauma, surgeries, or chronic conditions.
Separate $50K fine for resident’s death
In addition to the Class A licensure violation and $25,000 fine issued by IDPH during the second quarter, Allure of Galesburg faces a $50,000 fine following an investigation into a resident’s death.
In an April 2026 investigation, surveyors found the facility failed to maintain operable emergency equipment, leading to a resident’s death in December 2025. They found crash carts were covered in dust, missing critical supplies, and equipped with non-functioning suctioning equipment.
Staff were not trained on the equipment and could not clear the resident’s airway when she choked on vomit.
The facility’s medical director told surveyors the death possibly could have been prevented with working suction equipment, while its administrator said the facility did not investigate the resident’s unplanned death or report it to IDPH.
Chicago-based Allure Healthcare Systems purchased the Frank Street nursing home from Aperion Care in 2023 for $3 million.
The company operates two other nursing homes in Galesburg — Allure of Lake Storey, 1250 W. Carl Sandburg Drive, and Allure of Knox County, 280 E. Losey St.
